These Regulations may be cited as the Health Insurance (Amendment) (No. 2) Regulations, 2016. 2. The Health Insurance Regulations (2013 Revision), in these Regulations referred to as the \u201cprincipal Regulations\u201d, are amended in regulation 8(1)(a) by deleting the words \u201cin the Form in Schedule 2\u201d and substituting the words \u201cin a form published from time to time by the American Medical Association\u201d. 3. The principal Regulations are amended in regulation 9(2) as follows - (a) by deleting the words \u201cThe claim forms\u201d and substituting the words \u201cThe claim form\u201d; and (b) by deleting the words \u201ca HCFA 1500 or UB 92 form\u201d and substituting the words \u201ca form introduced and published by the Centres for Medicare and Medicaid Services (CMS) and the National Uniform Billing Committee\u201d. 4.
