SCHEDULE SL 50 of 2021 SCHEDULE (Paragraph 6) PARTICULARS TO BE INCLUDED IN RETURNS The particulars to be included in the returns in respect of every person to whom this Order applies shall be as follows \u2014 1. Head of household (1) List of household members (2) Housing (a) availability of \u2014 (i) internet at home; (ii) cable or satellite; and (iii) air-conditioning; (b) number of \u2014 (i) radios; (ii) televisions (iii) electric generators; (iv) fixed-line telephones; (v) mobile phones; (vi) computers; (vii) tablets or e-readers; (viii) motor vehicles; (ix) motorcycles or mopeds; and (x) watercrafts (including pleasure boats, jet skis, etc.); (c) tenure; (d) type of dwelling; (e) housing insurance; (f) number of bedrooms; (g) number of bathrooms; (h) shared bathrooms; (i) sewerage source; (j) water source; (k) drinking water source; SCHEDULE SL 50 of 2021 (l) shared kitchen; (m) fuel for cooking; (n) type of lighting; (o) produce renewable energy for own use; (p) produce renewable energy for sale; (q) number of household employees, including \u2014 (i) domestic helpers; (ii) gardeners; (iii) nannies; and (iv) caregivers (for sick child, sick adult or for elderly or disabled person); (3) Engagement in environmentally friendly activities. (4) Deaths (a) number of deaths in the last twelve months; (b) gender of deceased; (c) age of the deceased at death; and (d) place of death. (5) Emigration (a) resident status ten years ago; (b) lived outside of the Islands for one or more years continuously; (c) last country lived in; and (d) reason for migration from last country lived in. (6) Environment resource usage and food security (a) number of times in the past month household members engaged in agriculture and fishing for own consumption or sale; (b) number of household members engaged in agriculture and fishing for sale or donation in the past twelve months; (c) estimated value of agriculture and fishing in the past three months; and (d) number of household members who had no food to eat due to lack of resources in the past four weeks. 2. All persons (a) resident status; (b) relationship to head of household; (c) gender; (d) date of birth; (e) age; SCHEDULE SL 50 of 2021 (f) Caymanian and immigration status; (g) country of citizenship; (h) country of birth; (i) year last moved to the Cayman Islands; (j) mother\u2019s usual place of residence (i.e. country) at the time she gave birth to the respondent (i.e. person responding to census questions); (k) where applicable, mother\u2019s district of residence in the Islands at the time she gave birth to respondent (i.e.person responding to census questions); (l) usual place of residence one year ago (i.e. country or district in the Islands); (m) language most spoken at home; (n) religious denomination; (o) health insurance coverage; (p) school attendance; (q) type of school (i.e. primary, secondary, tertiary); (r) name of school; (s) mode of transportation to school; and (t) medically diagnosed illnesses. 3. Persons 5 years of age or older (a) difficulty seeing (even with glasses); (b) difficulty hearing (even with a hearing aid); (c) difficulty remembering or concentrating; (d) difficulty communicating or speaking; (e) difficulty with upper body function; (f) difficulty walking or climbing stairs; and (g) difficulty washing all over or dressing. 4. Persons 10 years of age or older Ability to read, write or understand a simple statement. 5. Persons 15 years of age or older (a) highest grade completed; (b) highest examination passed; (c) marital or union status; (d) employment status; (e) category of worker \u2014 (i) employee; SCHEDULE SL 50 of 2021 (ii) self-employed with no employees; (iii) self-employed with employees; (iv) unpaid family business worker; or (v) none of the above; (f) number of persons employed at place of work; (g) usual hours worked; (h) occupation; (i) name of business or industry; (j) address of business where work is carried out (including district); (k) mode of transportation to work; (l) earnings; (m) been a victim of crime; and (n) reported crime to RCIPS. 6.
