Revised this 27th day of January, 1998 Arrangement of Regulations Regulation FORMS Regulation 1 Citation These regulations may be cited as the Roads (Forms for Compensation) Regulations Forms (1) A notice of intention to claim compensation under section 9(1) shall be in Form A in the Schedule. FORMS FORM A (Regulation 3) NOTICE OF INTENTION TO MAKE A CLAIM FOR COMPENSATION UNDER SECTION 9(1) TO: His Excellency the Governor I hereby give you notice of my intention to make a claim for compensation in respect of the taking of land from me under the Roads Law (1998 Revision). Description of land: My interest in the land is as -YES NO (f) The name, mailing address and telephone number of any other person who occupies or has an interest in the land and specify the nature of his or her interest- (a) Details of items which may be affected by the proposed road and for which you consider you should be compensated (for example-land only, buildings, crops, trees, fences, TV dish.) CLAIM FOR COMPENSATION UNDER SECTION 9(2) TO: His Excellency the Governor I submit the following claim for compensation under the Roads Law (1998 Revision) in respect of the land specified below - (please print clearly) Telephone No. Description of land: Claimant’s interest in the land is as - YES NO (f) Other interest (please specify) The date and manner in which claimant acquired his/her interest in the land is- The name, mailing address and telephone number of any other persons who occupy or have an interest in the land and specify the nature of their interest Details of any public or private rights-of-way, easements or other incumbrances affecting the land Details (including block and parcel number, size and location) of any other land contiguous or adjacent to the land in respect of which this claim is being made in which you have a similar legal interest The date of the Gazette in which the section 3 declaration relating to the land was published

Consider the list below and put a dollar value on those items for which it is considered claimant should be compensated- (a) Market value of claimant’s interest in the land on the declared date- (b) Any damage sustained by reason of the taking of any standing crops or trees at the time of taking possession of the land- (c) Any damage sustained at the time of taking possession of the land, by reason of severing of such land from claimant’s other land- (d) Any damage sustained by reason of the dispossession injuriously affecting claimant’s other property or earnings- (e) Reasonable expense incidental to change of residence and/or business necessitated by the dispossession- (f) Any damage resulting from diminution of the profits of the land between the declared day and the time of taking possession of the land- (g) Any increase in the value of claimant’s interest in any remaining, contiguous or adjacent land which is likely to accrue from the use to which the land subject to the claim will be put- (h) Other costs or losses which should be considered in assessing compensation-

The claimant hereby elects to have his/her claim for compensation assessed as- (a) one-stage assessment, or (b) two-stage assessment. Claimant’s choice has been underlined and claimant acknowledges that this choice is irrevocable.
